Healthy Habits For All Survey Giveaway Official Rules
NO PURCHASE OR PAYMENT NECESSARY. To be eligible, all survey questions must be completed. Enter online at https://www.surveymonkey.com/r/9YH3JW9. Up to 200 fully completed surveys will be sent a $5 Starbucks e-card. All eligible entrants (up to 200) who submit a fully completed survey will constitute the winner of the Sweepstakes.
The Sweepstakes is open only to teachers in grades K–5 who are residents of the United States (one of the 50 states or the District of Columbia) and who have downloaded the Healthy Habits For All curriculum.
The winners will be sent their $5 Starbucks e-card by email. In the event of a dispute over the identity of an online entrant, entry will be deemed submitted by the holder of the email account.
Employees of WeAreTeachers, Healthy Habits For All, and members of their families living in the same household, and the program sponsor and their respective parent companies, subsidiaries, employees, brokers, distributors, dealers, retailers, affiliates, and their immediate families, and their respective advertising, promotion, and production agencies are not eligible to enter.
One entry per person; duplicate and/or mechanically reproduced entries will be disqualified. All applicable federal, state, and local laws and regulations apply. Odds of winning depend on the number of eligible entries. Void where prohibited or restricted by law.
Prize: Two hundred (200) winners will each receive one (1) $5 e-card to Starbucks.com (approximate retail value: $5) (the “e-card”). Taxes on the e-card, if any, are the sole responsibility of the winner.
All entrants, as a condition of entry, agree to abide by these rules and agree to release WeAreTeachers, Healthy Habits For All, and their respective parent companies subsidiaries, employees, brokers, distributors, dealers, retailers, affiliates, and their respective advertising, promotion, and production agencies from any and all liability for injuries or damages of any kind sustained through participation in this promotion and/or use or misuse of a prize once accepted.
No cash substitutions, transfers, or assignments of the e-card allowed, except by Sponsor in case of unavailability, in which case a prize of equal or greater value will be awarded. Sponsor’s decisions with respect to all matters related to this Sweepstakes shall be final.
Your responses to the survey questions will be provided to Healthy Habits For All in aggregate form and will be anonymous. Sponsor, Healthy Habits For All, and their parent companies subsidiaries, and affiliates, and their respective advertising, promotion or production agencies are not responsible for technical, hardware, software, or telecommunications malfunctions of any kind, lost or unavailable network connections, or failed, incorrect, incomplete, inaccurate, garbled, or delayed electronic communications caused by the sender, or by any of the equipment or programming associated with or utilized in this Sweepstakes that may limit the ability to play or participate, or by any human error that may occur in the processing of the entries in this Sweepstakes. If for any reason (including, but not limited to, infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other cause beyond the control of Sponsor, which corrupts or affects the administration, security, fairness, integrity, or proper conduct of this Sweepstakes) the online portion of the Sweepstakes is not capable of being conducted as described in these rules, Sponsor shall have the right, at its sole discretion, to cancel, terminate, modify, or suspend the Sweepstakes.
